IMMUNIZATION REQUIREMENTS FOR CHILDREN ENROLLED IN MISSOURI CHILD CARE AND PRESCHOOL FACILITIES: 4 DTP, 3 Polio, 3 Hep.B, 1 or more Hib (with at least 1 after the 1st birthday), 1 MMR, and 1 Varicella
2007-2008 SCHOOL YEAR IMMUNIZATION REQUIREMENTS: Pre-K; 3+ Dtap**, 3 polio, 1 MMR, 3+ Hib*, 3 Hep. B, 1 Varicella K-2 grade; 4+ DTaP**, 3+ Polio, 2 MMR, 3 Hep.B, 1 Varicella 3-4 grade; 4+ DTaP**, 3+ Polio, 2 MMR, 3 Hep.B 5-8th grade; 4 DTaP, 3+ Polio, 2 MMR, 3 Hep.B, Td booster*** 9-12; 3 DTaP, 3+ Polio, 2 MMR, 3 Hep.B, Td booster*** * atleast 1 after 1st birthday ** 4th DTap must be after 4th birthday, otherwise they will need a total of 5 DTap** *** Td booster may be given five years after the last DTaP, but is REQUIRED 10 years after the last dose*** If your child has had any new immunizations, please send a copy of the updated information so we can update their record.
